Added updated layout code. Didn't have the time to set up full dev environment so for now I just checked that this layout works as a standalone css + html and has the same input id's / selectors are there for the js code to select and parse. Quickly looked through the page.js as well and didn't see anything that would break the layout.
The resulting layout should look like this:
