Helper script: table of contents

The Markdown wiki doesn't support TOC generation and they are troublesome and error~prone to maintain manually. There are tools that do that and GH has addressed this issue on their web interface but not on wikis.

A script that could scan all Markdown files and generate (or update) tables-of-content in-line would be ideal.
