Loss of visibility of buttons based on screen resolution.

Describe the bug
The buttons at the bottom of dialogues - forms mainly used for adding tests, entities, categories etc. resize out of view based on screen resolution. Some of the dialogues also do not have the close button at the top.

To Reproduce
Steps to reproduce the behavior:

1. Go to configuration->categories->add category
2. Based on screen resolution, the buttons below the data entry fields could be out of view OR the close button (sample attached) at the top of the form could be missing.

Expected behavior
The forms to have all buttons in view and also have a close button at the top.
